doesn't your supervisor complete that part? For mine, he just put staff accountant for my Job title in question 8 and then for number 7 he just checked the applicable services performed. I don't see any where on the form where it asks for description of duties.

If you have access to your job description when you applied for your job, I'd use that. My employer just copied the job duties that were in the job posting last year.

I called the board and was told that wasn't good enough. I have to fill in the form with details, my employer has to verify my details, my employer has to then write their own letter that is attached with a job description and other information. The lady there said to include as much information as possible – the more the better.
