NYS Licensure forms. Need help.

    Hello,

    I passed all four parts of the CPA exam in NY and I am just filling out the forms to get my license. do you file Form 1 (Application for Licensure & First Registration) when you initially apply to take the exam or after or after you have passed all four parts? I remember filing a form and paying a fee to NYS when I initially applied to take the CPA exam but I can’t remember if it was on Form 1, or if it was another form.

    In addition, the only other forms we need are Form 4 and Form 4B?

    If anyone has an insight it would be greatly appreciated.

    If you haven't filled out form 1 previously along with sending the check, you need to do that. You also need to fill out Form 4 and have your employer or former employer fill out Form 4B. It all needs to go to the Office of the Professions. The exact address to mail is located on the forms.

    Once you send everything in, it will take anywhere from 12 – 16 weeks. One other bit of advice is to contact NASBA and make sure that they are sending your education forms to NY state as well as your last passing grade.

    Also make sure the 4B is signed an notarized and submitted by the CPA themselves. Also make sure they are a NY registered and active CPA.

    FAR - PASSED!
    REG - PASSED!!
    BEC - PASSED!!!
    AUD - PASSED!!!!

    Pending license - COME ON NYS BOA!
